  1. In large bowl, toss together pasta with pepper jack cheese and half the cheddar cheese and pour into greased 2 quart baking dish.
  2. In same bowl, mix together the flour, salt, black pepper, cayenne pepper, dry mustard, nutmeg, sour cream, egg, cream and half-and-half.
  3. Pour mixture over the pasta/cheese mixture and top with remaining cheddar cheese.
  4. Bake uncovered until top is just beginning to brown, about 35 minutes.
  5. Meanwhile, in a skillet over medium heat, melt butter and add bread cubes and toast until golden.
  6. Spread bread cubes on top of macaroni and cheese and continue baking until golden brown, about 10 minutes, more.
